Supplemental Application Forms

Thank you for submitting your forms.  You are almost done!   Two more steps.

 

Step 1 –  Please print out the following forms and submit them as required.  If you are not able to print, please send an email to Ms. Cardinale at admissions@aquinastars.org and we will mail the forms to you.

  • Supplemental Parent Information (for new families/new siblings applying) – This form needs to be completed and submitted either via email (admissions@aquinastars.org) or directly to the admissions office.
  • Parish Confirmation Form – If you are a Catholic family submit this to your parish.  Please note that the parish uses your financial contributions or volunteering to confirm your participation. Non-practicing Catholics or non-Catholic’s should complete and submit the form directly to the school office.
  • Student Recommendation Form (for new students to the school) – The Recommendation form is required for each student in grades PK-8th grade.  PK and Kindergarten students who have never been in school or daycare do not need to submit the form.  Please download the form and submit it to your child’s teacher as soon as possible.
  • School Entrance Health Form (for new families and current students in grades PK, K, 7).  All new students to the school are required to submit an updated Virginia State School Entrance Health Form.  Physicals may be completed out of state but physical information must be on the Virginia School Entrance Health Form.  Physical dates need to be within one year of the first day of school (August 2020-August 2021).

Step 2Pay the application fee (new students only)   ($75 for siblings of current student / $150 per family, regardless of the number of new students applying)

Step 3Pay the $100 Registration Fee.  (Current families only or new families after acceptance
