Supplemental Application Forms

Thank you for submitting your electronic application.  You are almost done!   Two more steps.

 

Step 1 –  Please print out the following forms and submit them as required.  If you are not able to print, please send an email to Ms. Cardinale at admissions@aquinastars.org and we will mail the forms to you.

  • Student Recommendation Form – This needs to be submitted to your student’s current teacher.  The teacher will return the form to the school.  PK and Kindergarten students who have not been in school do not need to submit this form.
  • Parish Confirmation Form – If you are a Catholic family submit this to your parish.  Please note that the parish uses your financial contributions or volunteering to confirm your participation. Non-practicing Catholics or non-Catholic’s should complete and submit the form directly to the school office.
  • Records Request Form – Submit to your current school.  PK and Kindergarten students who have not been in school or who have not been issued a progress report do not need to submit this form.

 

Step 2Pay the application fee   ($75 for siblings of current student / $150 per family, regardless of the number of new students applying)
